Cancellation/Rescheduling policy:
Please be aware there is a 24-hour cancellation policy. Appointments canceled with less than 24 hours notice will be charged at half of the full cost. If an emergency occurs and you cannot make an appointment this policy will not be in effect the first occurrence.

FEES
INITIAL CONSULTATION: $225.00 (2 hour initial consultation).
FOLLOW UP: $120/hour ($2/minute).


PHONE CONSULTATIONS:(for follow-up only) $120 per hour ($2/minute).
BRIEF PHONE CALLS: 5 minutes or less in duration - No Charge.


PEDIATRIC (12 or under) CONSULTATION INITIAL FEE: $100.00 (1 hour).
FOLLOW UP: $120/hour ($2/min).
BRIEF VISIT: $60.00 (15-30 minutes).



DETAILED FOLLOW-UP EMAIL RESPONSE TO YOUR QUESTIONS will be billed at $2.00/minute.
Invoice will be included with response (due upon receipt).
